"Get Out of Your Mind and Into Your Life" - Hayes

Somewhere along the way your priorites shifted from managing your life to managing your inner life.   Usually this "management" manifests itself as avoidance.  Avoidance of fear, rejection, boredom, etc.  This book is about how to realize that and how to accept these thoughts and still live the life that you want to live.   It goes into great detail about how the whole process works and what you can do to short-circuit the process and get on with your life.   The therapy is called Acceptance and Commitment Therapy (ACT).   The book is a workbook.   There is no instant cure.  There is however a pathway that leads back to a an active and engaged life.   This book will show you the path and the pitfalls.  How far get get along that path depends on you.

It is the first book I have seen that actually gives a good explanation of procrastination.   Highly recommended.   Do yourself a favor and get the book.
